More than half of office workers don't consistently get a good night's sleep

Home health aides have long had the distinction of being among the lowest-paid workers in the United States, but now it's revealed they also get the least amount of sleep.

Concern over finances and an overall stressful job may be keeping home health aides up at night; but lawyers also made the list of sleep-deprived occupations.?

A new study of professions that get the least and most amount of sleep was commissioned by a mattress company and based on a survey done for the National Center for Health Statistics.?The research found health aides, lawyers and police officers got the least? sleep, while loggers, hairstylists and sales representatives were the among the most-rested workers.

?We encourage people to take stock of their sleep habits and make improvements where they can,? said?Robert Oexman, director of the Sleep to Live Institute in Joplin, Mo., and a consultant for Sleepy's, the mattress company behind the study.

Here?s a rundown of the sleepiest workers based on average amount of sleep per night:

Overall, nobody seems to getting a lot of sleep no matter what they do. Even loggers, who topped the list, only clocked an average of seven hours and 20 minutes sleeping, compared with? six hours and 57 minutes for the sleepiest workers, home health aides.

Turns out, a lot. Two Harvard Medical School studies found that insomnia does a number on your productivity, and a lack of sleep can affect your health.?

You could make up the difference by napping at work. A 2011 study from the National Sleep Foundation and Philips Electronics found that one in four employees admitted taking a nap at work.

That study also found:

  • 85 percent of office workers say they could be more productive if they slept more.
  • More than half of office workers don't consistently get a good night's sleep.
  • Two-thirds of office workers surveyed said lack of sleep means their day begins on a low note.
  • Two-thirds of employees do not wake up before their alarm goes off and more than one-third are not ready to get up when their alarm goes off.

'Chemo Brain' May Linger 20 Years After Breast Cancer Treatment


MONDAY, Feb. 27 (HealthDay News) -- "Chemo brain," the name given to the mental fog and related memory problems that can occur during and after chemotherapy, may last for two decades after breast cancer treatment, new research suggests.

In the new study, 196 women with breast cancer who were treated with chemotherapy roughly 21 years earlier performed worse on tests of their memory, processing speed and other thinking ("cognitive") skills when compared to their counterparts who had never been diagnosed with cancer.

Participants had all been treated for breast cancer with a chemotherapy combination that included the drugs cyclophosphamide, methotrexate and 5-fluorouracil between 1976 and 1995. This regimen was considered the standard of care for breast cancer worldwide from the 1970s to the 1990s and was received by thousands of women during this time. Women in the study were aged 50 to 80.

"To our knowledge, this is the first study to suggest that subtle cognitive deficits may be among the long-term effects of chemotherapy, especially of the earlier regimens," study author Sanne Schagen, a group leader at the department of psychosocial research and epidemiology at the Netherlands Cancer Institute/Antoni van Leeuwenhoek Hospital in Amsterdam, said in a news release from the American Society of Clinical Oncology.

She added that while the results don't suggest that breast cancer survivors need to be watched more closely for memory and thinking problems, they could guide referrals to support services as needed.

In general, women who received this chemotherapy regimen had lower scores on tests of their ability to recall words, information-processing speed, and coordination of thinking and hand movement, such as putting pegs in a board, than women who did not. The results on these tests were similar to those seen among people who had just completed chemotherapy, and their magnitude was comparable to roughly six years of age-related decline in mental function, the study authors noted.

The researchers also assessed the women for depression and self-reported memory problems as part of the study. The women who had received chemotherapy also had more memory complaints than their peers who did not have chemo, but these complaints were not related to how they performed on memory tests.

The study findings were published online Feb. 27 in the Journal of Clinical Oncology.

Dr. Tim Ahles, the director of the Neurocognitive Research Laboratory at Memorial Sloan-Kettering Cancer Center in New York City, said this new study is the first to illustrate that long-term breast cancer survivors are still experiencing difficulty with their thought processes. "It adds to the notion that these effects may be long term and permanent," he said.

The effects of so-called chemo brain vary from person to person, and not all people who receive chemotherapy will develop or be affected by these issues.

"If someone is a professional who has an extremely demanding job, even a small change may be problematic, whereas for someone who is retired and has a more relaxed lifestyle, it may be less of a problem," Ahles said. "For women who are newly diagnosed and looking at treatments, it is still important to know that not everyone experiences these deficits."

In terms of prevention and treatment, there are more questions than answers right now. "This is an active area of research, and now that we have identified this is a real problem, people are turning their attention to what we can do to help treat and reduce some of the negative impact," Ahles noted.

Dr. Marisa Weiss, president and founder of Breastcancer.org and director of breast radiation oncology and breast health outreach at Lankenau Medical Center in Wynnewood, Pa., said the new study "shows only a few pieces of this complex puzzle."

She said it wasn't clear whether subtle losses in mental function were due to cancer alone, the effects of other therapies such as anti-estrogen treatments, or other factors.

Although the study uncovered an association between the cancer treatment and memory problems in the study patients, it did not prove a cause-and-effect relationship.

"There were many differences between these two groups," Weiss said, including whether or not they underwent the specific chemotherapy regimen. "We still need to fill in the other puzzle pieces before we can see what the picture is going to look like."

Dr. Stefan Gluck, a professor in the department of medicine and assistant director of the University of Miami/Sylvester Comprehensive Cancer Center, said that the new findings confirm this is not a temporary problem. "It may be the combination of drugs used 20 years ago were more likely to cause these issues," he said. "Today's drugs may be less likely to cause long-term cognitive effects."

He also added that drugs taken by many breast cancer survivors to stave off a breast cancer recurrence after treatment may also contribute to chemo brain.

(AP) ? The government of India's Andhra Pradesh state said Monday it would push prosecutors to move ahead with 76 criminal cases against employees of Indian lenders it believes were involved with driving overindebted borrowers to suicide.

The comments came in response to an Associated Press story that showed despite denials, top officials from India's SKS Microfinance had information that implicated its employees in borrower suicides. SKS is a market leader in India's microfinance industry, which specializes in small loans intended to lift up the very poor.

R. Subrahmanyam, principal secretary for rural development in Andhra Pradesh, said the story spurred state officials to ensure that the pending cases get swift attention. "We would like to take up the matter with the prosecution department," he said. "Usually this is not something which we would do."

"Once it goes to the court let's see how it goes," he said. "The minimum the state should be doing is compelling the prosecution to act quickly."

The government of the southern Indian state, which was India's largest microfinance market, blamed a spate of suicides in late 2010 on aggressive lending and collection tactics by microfinance companies. To date there have been no convictions and after scores of earlier arrests, no microfinance company employees are currently in jail, according to Andhra Pradesh officials.

The companies have denied responsibility for the deaths.

Davuluri Venkateswarlu, the director of Glocal Research in Hyderabad, was commissioned by an industry group, the Microfinance Institutions Network, to examine the role of microfinance companies in 44 deaths.

That report has not been made public, but Venkateswarlu said that in 13 cases there was evidence of a link between the actions of microfinance employees and the borrower's suicide or attempted suicide.

He said India's top three microlenders ? SKS Microfinance, Share Microfinance and Spandana Sphoorty Financial ? were implicated in the majority of those cases.

Share founder and managing director Udaia Kumar denied responsibility for borrower suicides, saying his company has always "upheld the best practices and client protection principles."

"We deny any link between our staff behavior and any deaths in the state of (Andhra Pradesh)," he said by email.

Spandana spokesman Nitin Agrawal said in an email that some Spandana employees have been acquitted and that the company is cooperating with local authorities in all ongoing investigations.

SKS officials say they stand by their September 2011 affidavit to the Supreme Court of India, in which chief executive M.R. Rao swore that SKS "is neither the cause of nor responsible for any suicides in the state of Andhra Pradesh." The company denies that its board of directors authorized any investigations that implicated its employees in some of the suicides.

SKS said its employees have been acquitted by courts in two cases and only one case remains ongoing. SKS said that in another 12 cases police determined they were not involved or absolved employees of responsibility during their investigations. Those 12 cases are on the government's list of 76.

Shares in SKS Microfinance have fallen 10 percent since Friday, despite the company's Monday announcement that it had arranged to sell 3.54 billion rupee ($72.0 million) worth of borrower loans to an unnamed government-run bank. SKS said it was the largest such securitization deal in "Indian microfinance history."

Agrawal of Spandana said the stories behind the suicides were riddled with political and emotional complexity.

He said local moneylenders, who lost business as microfinance companies grew, encouraged officials to unfairly blame the companies for borrower suicides, when in fact family stress and hounding by the moneylenders were to blame.

He said Venkateswarlu's research was tainted because of media sensationalism and vested local interests.

"The same cases had been interviewed so many times by so many journalists that sensationalizing the stories was the order for the day," he said in an email.

Venkateswarlu said that in speaking with people in the field he found that some cases had been resolved out of court, with companies paying the families of the dead to settle the matter.

In one instance, Share staff paid the family of the deceased to drop the case after they complained to the police, he said.

Share's Kumar denied giving payments to victims.

Today, police case files brim with forgotten, grim details, and microfinance lending has ground to a halt in Andhra Pradesh, after a state law clamped down on abuses by introducing onerous restrictions on loan disbursement and collection.

Spandana's Agrawal said that with the halt in microfinance lending, the poor of Andhra Pradesh have been driven back into the arms of moneylenders.

Just off the path before the tarn is Brother?s Parting Stone and much easier to spot from this direction.

A poem is inscribed into the stone though it is weatherworn and largely unintelligible. The stone is said to mark the spot where William Wordsworth and his brother John last saw each other before John drowned at sea.

As you reach Grisedale Tarn cross the stepping stones to the left and walk clockwise around the tarn towards Hause Gap and follow the clear path alongside the wall to the summit of Fairfield. Due to the rocky nature of the summit and various piles of stones, shelters and paths in many directions, a map and compass are essential to get off the summit safely in poor conditions.

There are many shelters on Fairfield for a lunch stop and are perfect to survey the view.? The next leg of the walk heads downwards over Cofa Pike a rocky pinnacle between Fairfield and St Sunday Crag.

From here don?t forget to look back to Dollywagon Pike across the valley for the impressive view of the craggy face.

The descent from St Sunday Crag offers several choices, a straight down descent from Birks to Elm How Plantation, however my preference is to walk down over Birks towards Trough Head and through the interesting rocky tops of Arnison Crags. This route takes you straight down into the garden and carpark of the Patterdale Hotel, which is a perfect place to stop for a well deserved pint or cup of tea in the welcoming bar.

There are several Lake District cottages in the area which are ideally situated to give you a great base from which to explore Grisedale and other surrounding valleys.

(AP) ? Revelers hit the streets Tuesday to celebrate Mardi Gras, lured to the French Quarter and stately oak-lined avenues by the chance to snag beads and baubles from seemingly endless parades in the final unfettered party before the somber season of Lent.

The French Quarter was full of costumed revelers. Wearing a bright orange wig, a purple mask and green shoes, New Orleans resident Charlotte Hamrick walked along Canal Street to meet friends.

"I'll be in the French Quarter all day," Hamrick said. "I don't even go to the parades. I love to take pictures of all the costumes and just be with my friends. It's so fun."

Brittany Davies of Denver was struggling through the early morning hours. Still feeling the effects of heavy drinking from the night before, her friends had her out again early Tuesday.

"They're torturing me," Davies joked. "But I'll be OK after a bloody Mary."

At the Superdome, the predominantly African-American Zulu krewe loaded their floats with their signature decorated coconuts, a parade crowd favorite. Most were in the traditional black-face makeup and afro wigs krewe members have sported for decades.

Meanwhile in the Garden District, clarinetist Pete Fountain prepared to lead his Half-Fast Walking Club on its annual march down St. Charles Avenue, the traditional start of the daylong series of parades.

After Zulu, the parade of Rex, king of Carnival, would make the trek down the avenue and to the city's business district, with hundreds of thousands of people pleading for beads and doubloons.

Fountain, 82, gave a "thumbs up" to start off and his band launched into "When The Saints Come Marching In" as they rounded the corner onto St. Charles Avenue shortly after 7 a.m. It was the 52nd time that Fountain's group has paraded for Mardi Gras. This year, the group wore bright yellow suits and matching pork pie hats for its theme, "Follow the Yellow Brick Road."

Along St. Charles, groups of people, many in costumes, breakfasted as children played in the street. Small marching groups were already on the move. The Skeleton Krewe, 25 people dressed in black skeleton outfits, were on their way to the St. Louis Cathedral.

Tom White, 46, clad in a pink tu-tu, bicycled down the avenue with his wife, Allison, on their way to the French Quarter. "I'm the pink fairy this year," said White. Allison White was not in costume. "He's disgraced the family enough," she said of her husband.

But Tom White was in the spirit of the day. "Costuming is the real fun of Mardi Gras. I'm not too creative but when you weigh 200 pounds and put on a tu-tu people still take your picture."

The stakeout for prime spots along the Mardi Gras parade route started Monday, with legions of Carnival die-hards jockeying for the best places to vie for beads thrown from floats on Fat Tuesday.

Stephanie Chapman and her family had set up in their usual spot on the St. Charles streetcar tracks. They'd arrive at 4 a.m. on Tuesday and would be staying for the duration. "This is a beautiful day and we'll be here until it's over. It won't rain on my parade, But if it does I won't pay any attention," she said.

Across the Gulf Coast, Mardi Gras was getting into full swing. In the Cajun country of southwest Louisiana, masked riders were preparing to go from town to town, making merry along the way in the Courir du Mardi Gras. And parades were scheduled elsewhere around Louisiana and on the Mississippi and Alabama coasts. The celebration arrived in Louisiana in 1682 when the explorer LaSalle and his party stopped at a place they called Bayou Mardi Gras south of New Orleans to celebrate. The site is now lost to history.

On Monday night, the Lundi Gras celebration culminated with the parade of the Krewe of Orpheus, led by entertainers Harry Connick Jr. and Hilary Swank. Rocker Bret Michaels and Grammy-winning singer Cyndi Lauper ushered in Mardi Gras with musical performances in the wee hours of the morning at the bash that followed the parade.

Revelers in costume danced and posed for pictures with Mardi Gras Indians wearing elaborate costumes with feathers and beads.

The end of Mardi Gras gives way to the beginning of Lent, the period of fasting and repentance before Easter Sunday.

Iraqis profit from Syrian arms smuggling (AP)

BAGHDAD ? The rifles are first taken apart and hidden in cigarette cartons and kerosene tanks. Younis al-Lehaibi and his sons then divide them into their trucks and head out to Iraq's vast, dusty border with Syria. Their objective: to smuggle weapons to Syrian rebels who seek the overthrow of President Bashar Assad.

It's a turnaround from the height of the Iraqi war six years ago, when weapons and fighters would cross from Syria to aid fellow Sunnis in Iraq.

Mindful of roaming border police, al-Lehaibi ditches the trucks once in Syria and travels the rest of the way on donkey. The Kalashnikovs are put back together, cleaned and handed over to a boy who hands him cash and brings the weapons into a nearby village.

Al-Lehaibi, 46, carefully examines his payment ? usually $1,000 in U.S. dollars ? to make sure the currency isn't counterfeit. And then he slips back into Iraq ? eight hours after he left his home in the city of Mosul, 225 miles (360 kilometers) northwest of Baghdad.

Until a month ago, al-Lehaibi said, it wasn't worth the effort. There wasn't much profit in smuggling Kalashnikov rifles. The Syrian rebels had all the weapons they needed.

The spike in demand likely reflects how Syria's uprising, as it approaches its first complete year, has transformed into an outright clash of forces as the opposition turns more to armed action. In another dangerous turn, more foreign fighters, possibly linked to al-Qaida, are believed to be crossing from Iraq to join the uprising against Assad.

The top U.S. intelligence official, James Clapper, said Thursday that a series of bombings against the Syrian regime in recent months in Damascus and Aleppo bear "the earmarks of an al-Qaida-like attack."

For al-Lehaibi, pale man with reddish hair, the jump in demand for weapons means business has never been better.

"It's about making a good, profitable business," al-Lehaibi said, describing his weekly smuggling trips in an interview with The Associated Press.

He hastily added: "It's also to help the Syrian people topple the tyrant who suppresses his nation for decades with severe brutality."

A senior Iraqi security official in Baghdad told AP that intelligence over the last four months has revealed a flow of al-Qaida-linked fighters from Mosul into Syria, including two militants trained as suicide bombers. A Mosul police official confirmed the increase in gun smuggling to Syria but described it as limited.

Both officials spoke on condition of anonymity because they were not authorized to release the information.

Earlier this week, al-Qaida's global leader Ayman al-Zawahri called on Muslims from Iraq, Jordan, Lebanon and Turkey to join the Syrian uprising, which began in March as a peaceful protest but has grown into a bloody insurgency. Experts fear Syria will follow the same path Iraq charted just a few years ago: teetering on the edge of civil war, only to face an indefinite future of instability and, for many, despair.

A Kalashnikov rifle, known as an AK-47, usually sells for no more than $200, al-Lehaibi said. But their newfound demand has encouraged dealers to raise the wholesale price to about $700. Al-Lehaibi acts as the middle man, making a tidy profit by charging $1,000.

The prices jumped so much that even al-Qaida leaders in Iraq chided dealers for price gouging in a statement on a militant website the day after al-Zawahri's edict.

"The arms brokers have raised the prices very high for the Mujahedeen who are trying to transfer that to their fellow rebels in Syria," read the statement posted on the site associated with the Islamic State of Iraq, which is an al-Qaida splinter group. "Unfortunately there are gangs that claim they affiliate with the jihadist, but they benefit from such trade.

"Please make a special effort in this period for the continued flow of arms," the statement urged.

Smuggling routes have existed for generations across the porous Iraqi-Syrian border, which stretches for 363 miles (605 kilometers). From the main legal border crossing in the Iraqi town of Rabiya, the lights of the Syrian village al-Yarubyah can be seen twinkling just a few miles (kilometers) away.

But most of the border is empty and marked only by a dirt berm with no signs or fences separating the two nations.

Al-Lehaibi has been smuggling for more than 10 years. He began sneaking food rations into the autonomous Kurdish region in northern Iraq in the 1990s while Saddam Hussein was in power. Later, he smuggled satellite receivers, cigarettes and kerosene between Syria and the Kurdish region.

Sometimes al-Lehaibi trades his guns for sheep ? which gives him a convenient cover as a sheep dealer in the rare times border police have accosted him.

"We do fear being caught, but a fundamental principle of our work is to put fear behind us," he said. "There are tighter security measures in Mosul, but there are dozens of smugglers who do this job after years of relations and help from bordering villages who have these needs."

Al-Qaida and Syria's homegrown opposition appear to have very different agendas. Syrian rebels have for the last 11 months demanded a more democratic system for the country, and even liken the Assad regime to al-Qaida.

The Assad regime's "acts in torturing and killing its opponents are very similar to those used by al-Qaida members in annihilating anybody who disapprove with their dark believes and ideologies," the Local Coordination Committees, one of the main Syrian activist groups, said in a statement this week. During a hearing Tuesday, prosecutors argued he should be held. They say he bought a one-way ticket to Turkey, leaving his family behind.

However, defense attorneys say the government's evidence doesn't show the entire context of what was going on in his life and his reasons for trying to go to Turkey.
